Does this constitue falsifying medical records?

I work as a CNA at a nursing and rehab facility. There is another cna that works with me pretty regularly and she is just horrible to be blunt about it. Anyways, getting to the point. Tonight she filled out a shower sheet claiming to have given a resident a shower, and also charted it electronically. Only problem is she never actually gave them a shower!! It has been reported to the charge nurse, adon, don, etc. I'm just wondering if this is considered falsification of medical records? I tried to google it with no such luck. Thanks!!
